Accouchement à domicile : histoire d'une naissance heureuse.

Ceci est mon témoignage, ma vision de notre accouchement à domicile.
Pour des raisons de confidentialité, les prénoms ont été changés.
Dimanche 19 juin 2011.
Une petite main me gratte doucement le dos. Une petite voix d'ange me glisse - "je crois que ça a commencé". Il est 2h, nous sommes à l'aube de la fête des pères. Nous allons enfin voir la tête de notre petite Romane.
Neuf mois que l'on attend ce moment. Presque huit à préparer un accouchement à domicile.
Au départ j'étais assez réticent à cette idée : s'il se passe n'importe quoi de grave ? Si ça tourne mal ?
Heureusement, notre voisin est pompier et le médecin est à trois maisons d'ici. Et même si une naissance en hélico est vraiment trop class, je pense qu'on pourrait se passer de ce genre d'aventure.
Au fil des mois, mon petit coeur a su trouver les mots et me faire adhérer à son projet de naissance à domicile. 95% des accouchements se passent généralement bien et il est vrai que l'hôpital n'est pas l'endroit rêvé pour une naissance : aspiration, froid, bruits, agitations. Je fini par être convaincu, comme elle, qu'un accouchement à la maison sera mieux pour tout le monde, au chaud, dans le cocon tranquille de notre foyer.
Depuis plusieurs mois donc, on prépare cette arrivée. Près de chez nous une sage-femme à domicile vient de cesser cette activité. Force est de constater qu'elles ne sont pas très nombreuses à exercer des accouchements extra-hospitalier. Heureusement, nous avons des RDV réguliers avec Marie et Emilie, un cabinet de sage-femme à 1h30 environ de la maison.
La première rencontre avec Emilie nous met en confiance. Sa voix douce, sereine, à l'écoute, nous avait conquis. Le surcout financier de 700 euros s'expliquait par le fait que Marie avait souscrit une assurance professionnelle relativement élevée, mais dont Emilie bénéficiait.
Cette pratique d'accouchement, très majoritaire dans les pays du nord de l'Europe, ne trouve pas beaucoup d'écho en France et il semble compliqué de la mettre en place dans notre système de santé. Il est tout aussi étonnant de voir combien on ferme de maternités sans favoriser de système alternatif. Mais c'est un autre sujet...
Qui dit accouchement à domicile, dit aussi solution de repli en cas de soucis. Emilie et Marie pourraient disposer d'un plateau technique, mais à 2h de la maison. Pour nous c'est trop loin et décidons de visiter la maternité de notre gynécologue. L’occasion faisant le larron, nous en profitons pour rédiger un projet de naissance.
L'accueil ne fut pas enthousiaste. Même si la Loi prévoit la rédaction d'un projet de naissance, dans les faits c'est beaucoup moins évident et nombre de sage-femmes sont réticentes à l'idée même qu'on puisse avoir un projet.
Quelque peu dépité par l'accueil à cette maternité, nous décidons que nous nous rapprocherons de l'hôpital le plus proche, et faisons les démarches à l'identique d'une prise en charge hospitalière : Inscription, visite et information, RDV avec l'anesthésiste, etc.
Parallèlement, nous avons la chance d'avoir en bas de chez nous une sage-femme qui pratique l'haptonomie. Mais malheureusement pas les accouchements à domicile ! Le contact est plutôt bon, même si j'ai parfois le sentiment qu'elle me prend un peu pour un extra-terrestre. Nous faisons les séances d'hapto ensemble et j'en ressors totalement bluffé : Romane suit mes mains, se déplace en fonction de là où je les pose. Faut dire qu'elle bouge à fond dans cet utérus cinq étoiles. Et puis, l'haptonomie aide bien à soulager sa maman. Franchement, on est ravi et on attend le jour J.
Donc voilà, ça y est ! Le grand moment est enfin arrivé ! C'est aujourd'hui et c'est la fête des pères.
La consigne de la sage-femme était : "- Tu nous appelles dès que tu as des contractions toutes les cinq minutes et durant au moins une heure trente."
A cinq heures, on était bon. A 6h10 on appelle Emilie. Les sage-femmes se relayent tour à tour, et cette semaine l'accouchement se fera avec Emilie.
Je descends acheter des croissants à la boulangerie (bien meilleurs depuis le changement de proprio). A mon retour, Emilie est arrivée. Nous prenons ensemble le petit-déjeuner au soleil sous la pergola. Elle nous explique que finalement, nous l'avons appelé un peu trop tôt, et que c'est juste le pré-travail qui vient de commencer. Ce pré-travail peut durer des heures avant le début du "vrai" travail. Le monitoring indique que le bébé va très bien, et que le col est légèrement dilaté à un centimètre. Emilie a des amis tout près d'ici et décide de déjeuner avec eux, le temps que ça avance de notre côté.
De retour à 15h, elle teste le col et il est à presque deux centimètres. Les contractions sont supportables.
Nous sommes dans la chambre du bas, aménagée pour l'accouchement. Mon petit coeur va bien, on est en confiance, on fusionne, on va bientôt voir la tête de cet enfant. Nous bavardons avec Emilie entre deux contractions, sur comment s'est passé la grossesse, ces douleurs aux reins depuis plusieurs mois et sur ces démangeaisons qui sont survenues aux pieds et aux seins il y a quelques jours.
Emilie s'interroge. Des démangeaisons aux seins et aux pieds ? Ouuuulaaaaa ! Ca pourrait être un problème hépatique, une cholestase. Ca pourrait être grave, pour la mère comme pour le bébé.
Nous la rassurons : ma petite chérie a traversé toute sa grossesse avec tout plein d'envies. Là une orgie de cornichons, là elle a dévalisé tous les haricots-vert chez Carrefour, et la semaine dernière elle a mangé une boite de foie gras par jour. Sans parler des aller-retour de saucisse au roquefort. Alors forcément, une semaine charcuterie pour une fondue de légumes bio, ça fait un stress alimentaire.
On en est sûr, ma chérie va bien, le bébé va bien, tout va bien. Et puis une cholestase, ça fout un peu la jaunisse. La seule chose qui ai changé depuis 9 mois, c'est le poids. Le teint de ma petite femme n'a jamais été aussi beau. Donc, on se calme tout va bien !
" - Non, je veux faire des analyses aujourd'hui même. Il me faut les résultats aujourd'hui !
" - OK, c'est Dimanche aujourd'hui. Les labos sont fermés. Comment peut-on faire ?
" - On l'emmène à l'Hôpital, ils feront les exams."
Pour tout dire, une naissance à domicile qui commence à l'hôpital, c'est pas vraiment ce qu'on voulait. Déjà, pourront-ils faire ces analyses ? Emilie appelle la maternité, et la sage femme de garde lui répond que les analyses (Transaminase, TGO, TGP) doivent être complétées par des analyses de sels biliaires, et que si on vient pour effectuer ce type d'analyses, ils vont automatiquement hospitaliser. On raccroche, on laisse tomber l'hôpital.
Argggg !! Ca part en sucette. Tout allait bien, on était dans notre cocon, tout se déroulait à merveille et à son rythme.
Voilà qu'Emilie stresse. Elle tente d'appeler sa collègue pour un avis. Puis un ami gynéco. Quelqu'un confirme par téléphone qu'il faut bien les sels biliaires mais que l'analyse prend 3 à 4 jours. D'ici là, Romane sera née.
On la rassure à nouveau. Pas de jaunisse, pas de démangeaisons actuelles, et le bébé est au top. On se calme !
Emilie n'en démord pas :
" - Non, je veux ces analyses aujourd'hui"
" - Ok, je te propose, tu fais la prise de sang, je les apporte à l'hôpital et si les résultats ne sont pas bons, on transfère".
" - D'accord, on fait comme ça, mais je suis pas équipé pour les prélèvements."
On rappelle l'hôpital pour connaitre le type de tube qu'il nous faut : - "Un avec le bouchon vert, hépariné" !
Pendant ce temps, le pré-travail s'est arrêté. L'adrénaline à tendance à se transmettre rapidement. Mon petit coeur n'a plus de contraction.
Je file chez le voisin demander si sa belle-soeur infirmière peut nous dépanner d'un tube.
Pendant ce temps là, Emilie se calfeutre à nouveau dans sa voiture pour passer des coups de fil.
Je bondis dans la mienne direction le domicile de l'infirmière. Sympa, elle me donne le kit complet : de quoi piquer et une série de tubes, dont le fameux à bouchon vert hépariné.
De retour au bout d'une vingtaine de minutes je trouve Emilie et ma petite chérie dans le salon, assises face à face et l'air grave. La tension est visiblement montée d'un cran durant mon absence.
Emilie ne gère plus son stress et part au tapis. Elle nous indique d'abord qu'elle ne sent plus du tout cet accouchement à domicile, que d'après sa déontologie elle n'a plus le droit de prescrire ces analyses car elles sont "limite médico-légale" et qu'en plus elle n'est pas assurée en cas de pépins.
Il est 19h, elle nous fait flipper depuis 15h! On a proposé des solutions pour ces problèmes d'analyses, mais à chaque fois qu'on solutionne un problème, son discours change. Donc voilà, elle nous abandonne et nous annonce qu'elle n'est pas assurée : Ca y est, nous aussi on est dans le stress. Mais qu'est ce que c'est toutes ces conneries ????
Je réalise que l'on vient juste de se faire sortir de notre cocon à grand coup de pompe dans le cul !
Je ne cède pas à la panique, il faut absolument que je reste solide pour ma belle.
Mais mon petit coeur, craque !! Fond en larme. Je l'encourage, la rassure :
- "Ok, elle nous plante. On va aller à l'hosto. Si c'est grave et que la suspicion est avérée alors on hospitalise, sinon retour à la case départ. Allez on y va, plus vite parti, plus vite revenu."
Emilie rédige un note à l'attention de la sage-femme de l'hôpital, et retourne diner chez ses amis. Elle nous laisse leur numéro (elle n'a pas de chargeur de portable dans sa voiture) et nous on part, direction la maternité.
Vers 20h30 nous arrivons à l'entrée de la maternité. La sage-femme de garde est plutôt sympa, mais nous interroge un peu sur ce qu'on fait là…
On lui raconte alors notre choix de l'accouchement à domicile, que le pré-travail à commencé à 2h ce matin, qu'il s'est arrêté vers 16h, et que surtout il y a suspicion de cholestase.
Elle monitor le bébé, prend la tension et fait le prélèvement. Durant l'attente des résultats du labo, elle nous fait part de son avis : A priori, il n'y a pas de soucis, les démangeaisons étaient sur les seins et les pieds, mais pas sur les mains. Et puis, il n'y a pas de jaunissement visible et à 2 jours du terme, c'est pas déconnant d'avoir un léger dysfonctionnement hépatique d'autant qu'il y a eu un stress alimentaire… Mais attendons plutôt les analyses.
Les analyses confirment qu'il n'y a rien d'anormal si prés du terme. De toute façon les résultats des sels biliaire arriveront après l'accouchement et en plus ça se prélève à jeun. Puisque notre dossier était prêt dans cette maternité, elle nous propose d'accoucher ici, mais ne voit aucune contre-indication à un accouchement à domicile.
La sage-femme appelle Emilie chez ses amis pour lui donner son compte-rendu et lui annonce qu'elle nous libère pour accoucher à la maison.
Non décidément l'hôpital n'est pas l'endroit où l'on veut être, retour à la case départ, on prend congé direction la maison.
Dans la voiture les contractions reprennent à un rythme régulier et le doute s'installe.
Comment peut-on raisonnablement avoir encore confiance dans une sage-femme qui ne gère pas son stress, qui se documente en s'isolant avec son portable, qui semble ne pas maitriser son sujet et plus grave à mon sens, qui se range au dernier avis reçu par téléphone.
On savait depuis le début qu'il n'y avait rien de grave. Jusqu'à présent on était bien, tout se déroulait à merveille.
Pour moi la confiance est rompue, on ne peut plus lui confier cette mission.
Elle n'a pas été claire sur l'assurance ; laisse à penser qu'elle manque d'expérience; qu'est ce qui va se passer si ça tourne mal pour de vrai ?
On convient qu'à notre arrivée à la maison on lui proposera, si c'est possible, de demander à Marie de prendre le relais.
Emilie revient de chez ses amis en même temps que nous arrivons à la maison. Je m'éclipse de façon à ce que la conversation s'amorce entre filles. Au bout d'un moment je me rapproche. Emilie me dit qu'elle reste, inutile d'appeler Marie, elle se sent de nouveau en confiance sur cet accouchement. Nous plus vraiment. Ma belle est épuisée, je suis épuisé, les contractions recommencent, je vais devoir m'occuper à nouveau de mon petit coeur. Et puis si ça tourne mal, c'est pas grave, maintenant ils nous connaissent à l'hôpital.
On rentre dans la chambre du bas, Emilie dort en haut dans la notre.
Lundi 20 juin
Il est tard, la journée a été rude, on va essayer de trouver un peu de sommeil entre deux contractions.
Le matin on s'est un peu reposé; on a dû dormir 2h maximum. Emilie est partie acheter un chargeur de mobile et porter les prélèvements pour les analyses de sels biliaires faite à jeun sur prescription de la Miss-sage-femme d'hier soir (nous apprendrons plus tard que la sage-femme de garde est Miss de je-sais-plus-quoi).
Je masse régulièrement ma belle avec un mélange d'huile d'amande douce et d'huile essentielle de lavande. Je la câline le plus possible et lui pose une bouillotte à chaque contraction, ça semble bien la soulager. On cherche des positions permettant de réduire la douleur : à quatre pattes avec plein de coussins sur le ventre, assise les jambes tendues, mais la meilleure semble être debout.
A midi ça fait tout de même 34h qu'on a démarré l'aventure. On déjeune sous la pergola. Emilie revient de ses courses en début d'après-midi. Un premier toucher annonce le col à 2cm. C'est clair, ça n'avance pas vite.
Vers 15h les contractions se font plus douloureuses. Je masse, je colle des bouillottes… La poche des eaux fuit. Elle n'a pas cédée, il s'agit seulement de légères pertes. On s'installe dans différents endroits de la maison. Contre le mur du couloir on décide de se concentrer pour dire au bébé d'appuyer pour ouvrir le col. Si j'arrive à visualiser mentalement le bébé, je n'arrive pas à communiquer avec. On est tous fous : les contractions s'accélèrent. On sent que ça vient. Je la regarde : on retrouve ce premier regard échangé il y a sept ans.
Il est 17h, c'est dur. Un autre toucher indique que le col reste à deux. Rien ne vient. On ne comprend pas pourquoi c'est si douloureux et pourquoi il ne se passe rien. La poche fuit toujours.
On se fout de savoir si les voisins vont entendre les gémissements et on va se poser dans le jardin. Il fait chaud. Ca devient encore plus dur, je me sens impuissant à atténuer la douleur malgré mes caresses, les compresses, les massages.
Emilie part diner chez ses amis en attendant que ça avance.
Toute la soirée il y a des contractions, terriblement douloureuses, mais le col ne bouge pas, reste figé à la position d'hier.
Mon petit coeur prends un bain. Elle dort 20mn. Depuis environ 42h qu'on a commencé, il n'y a aucun progrès significatif. Nous sommes tous les deux lessivés physiquement, essorés moralement.
De retour, Emilie sonde le col, il est toujours à 2cm, malgré cette journée de travail supplémentaire. Mon amour craque ! Elle ne comprend pas pourquoi ça reste coincé au niveau du col alors que les contractions la font horriblement souffrir.
Elle n'en peut plus, s'effondre en larme.
-"je veux qu'elle sorte de moi !" lance t-elle avec ces tripes.
C'est le signal que là, les limites physiques et morales ont été atteintes. Je lui propose de partir à l'hôpital car maintenant c'est plus tenable.
Emilie nous alerte sur le fait que la poche est perforée depuis le début d'après-midi et qu'elle va recevoir une injection d'antibio. Elle nous conseille de dire qu'elle ça fuit seulement depuis une heure.
Nous préparons un sac avec quelques vêtements, des serviettes et les affaires de la petite et prenons la direction de l'hôpital à deux voitures.
Nous arrivons vers 23h30, il fait nuit devant l'entrée, seule les lumières de sécurité subsistent. Un homme nous attend, puis s'avance vers nous :
- "Bonsoir, je suis Julien, et je suis la sage-femme".
On savait que des hommes essayaient de conquérir cette profession à 98% féminine mais on n'en n'avait jamais vu. Voilà donc un spécimen. En même temps, nous ne sommes pas là pour faire de l'ethnologie, et passons rapidement à autre chose.
On lui résume la situation. On est déjà venu hier soir, et on n'a pas l'air comme ça, mais on n'est pas des beatniks. L'homme-sage-femme propose une injection de morphine pour calmer la douleur en espérant que ça fasse aussi tomber le stress afin que les contractions soient efficaces.
Nous voilà dans une chambre, tuyauté, monitoré, avec un petit fix qui se prépare. A peine injecté, la réaction est immédiate :
- "Tu sais, ça fait bizarre… C'est comme si je voyais dans une fenêtre d'erreur d'IPCOP. Tu sais, les bandes rouges autour, quand tu fais une mauvaise règle."
Le produit semble sympa, mais il y en a juste pour elle. On n'est pas des beatniks mais on est bien des geeks.
Malgré un gros mensonge, les antibios sont tout de même injectés.
Emilie prend congé et rentre dormir chez nous.
On se dit qu'on a bien fait de venir, que les douleurs s'estompent et qu'on va essayer de dormir.
Je me colle contre elle et on essaye de dormir dans ce petit lit.
Mardi 21 juin.
Il est trois heures environ lorsque l'homme-sage-femme revient faire un tour pour voir comment ça avance. Décidément, c'est un métier de patience.
C'est véritablement chiant le nombre de fois où des gens en blouses bleues ou blanches, viennent vous emmerder dans la chambre.
Ca fait près de 50h qu'on ne dort pas et y a toujours quelqu'un pour venir voir un goutte à goutte, contrôler le monitoring, ou faire encore un autre truc. L'hôpital semble sécurisant, mais pas du tout paisible. Dans la brochure d'accueil, il est dit que les papas sont merveilleusement traités, alors j'en profite pour demander un lit d'appoint.
A 7h du matin, l'homme-sage-femme revient faire le point avant de finir sa garde. Les contractions continuent, si tout va bien, on posera une péridurale en début d'après-midi, si le col est à 3cm. Je file en ville faire vite fait une reconnaissance préalable à l'état-civil (des fois que je meurs entre maintenant et l'accouchement prévu cet après-midi).
A 14h, ça y est nous voilà en salle d'accouchement. Elle est rose. Il y en avait une bleue, mais comme on a dit que c'était une fille, ils ont pensé bien faire.
Je me fais sortir - "C'est le protocole" - durant la péridurale par la sage-femme du moment. Tient, aujourd'hui c'est une femme. Et elle est plutôt sympa. J'occupe le temps à faire des pitreries avec la caméra de l'iphone. Il n'y a bientôt plus de douleur, c'est super cool.
On lui injecte du Syntocinon (c'est de l'ocytocine de synthèse) afin d'accélérer la dilatation du col. Et puis aussi un peu de Spasfon. On nous explique que les médecins trouvent que ça ne sert à rien, mais les sages-femmes pensent l'inverse.
Il est maintenant 16h, et le col est à 6cm. On se dit que ça progresse rudement bien. Hier on était à 1cm/48h, ça laissait entrevoir une libération autour du 14 juillet et on trouvait ça un peu lointain.
Si tout va bien et à ce rythme, à 19h on devrait voir la tête de la petite Romane.
A 18h45 Christian, le futur parrain, passe m'apporter un bout de quiche dont il est très fier, quelques sandwichs et trois bricoles pour me changer. L'autre soir, j'ai pensé à tout sauf à moi. Dans le SMS je lui demandais de me filer des trucs neutres, pas du type maillot du Bayern de Munich. J'ai été exhaussé : me voilà avec un sweet orange des Wallabies.
Il est 20h. Je croise l'homme-sage-femme en train de fumer une cigarette avant sa garde de nuit. Je me souvient qu'il s'appelle Julien et lui annonce que ça va être avec lui que ça va se passer. Le col est presque à 10cm, mais Romane ne sort pas.
Dans l'après midi, la poche des eaux s'est rompue lorsque sa collègue faisait un toucher pour mesurer le col.
Nous revenons ensemble en salle d'accouchement. Mon petit coeur trouve que c'est un peu long maintenant.
Ce garçon est curieux et nous pose des questions sur les motivations qui nous ont amené à un accouchement à domicile. Il évoque, pelle-mêle, le risque pour l'enfant, le risque pour la mère, la distance par rapport aux secours, etc. Il vend bien sa soupe le bougre. Parce qu'en plus d'être curieux, il a aussi l'air d'être malin.
Du coup, comme de l'empathie se crée, on en profite pour exposer nos demandes : pas d'aspiration, peau à peau dans les premières secondes de vies, pas d'épisio, pas de section du cordon durant les minutes où il bas encore…
Voilà notre sage-homme, reprenant une à une nos demandes :
- "On laisse les tissus se déchirer mais jusqu'à un certain point. Après il vaut mieux trancher. On n'est pas des excité du bistouri, on ne coupe pas par plaisir. Le peau à peau on le pratique déjà; on a vachement évolué".
Nous : -"On ne lavera pas le bébé de suite pour le peau à peau ?"
Lui : -"et s'il est plein de sang, on vous le pose dessus quand même ?"
Nous : -"heuuu… oui"
Lui : "- Vous savez, c'est pas pour me vanter, mais vous avez de la chance d'être tombé sur moi. Certaines de mes collègues ont des boutons dès qu'on leur parle de projet de naissance."
On lui laisse une chance de nous démontrer qu'on a de la chance : Julien semble vouloir concilier nos demandes et ses exigences professionnelles et/ou déontologiques. Il nous explique alors que oui, c'est possible de ne pas passer de sonde dans les narines. Pour cela il suffit d'en boucher une et de mettre un bout de verre devant le nez pour voir le souffle. Bon point pour lui, il n'est pas obtus l'animal. En revanche, comme la poche est rompue depuis de nombreuses heures, il veux faire un prélèvement dans l'estomac, pour vérifier que des bactéries ne s'y soient pas développées. Il nous rassure en montrant comment il va s'y prendre. On valide qu'on est d'accord. Concernant le cordon, il fait partie de l'école de ceux qui pensent qu'il peut y avoir échange sanguin entre le bébé et la placenta. On négocie qu'on verra le moment voulu. On a bien compris qu'il était conciliant et ouvert au dialogue, mais qu'il n'hésiterait pas à intervenir dès qu'il sentirait le moindre danger.
Entre 21h et 23h, le bébé ne descend toujours pas alors que tout va bien. Tout le temps monitoré, Romane n'est pas en souffrance.
L'avantage d'un homme-sage-femme qui fume, c'est que je peux faire le point assez régulièrement avec lui sans que mon petit coeur n'entende nos conversations.
Nous voilà donc dehors, sur le banc clop au bec, en train de voir quelles sont les pistes.
Il est 23h, on évoque maintenant la césarienne. Je partage son avis : au bout d'un moment il faut que tout le monde aille bien.
Je lui propose de voir si un changement de position peut faciliter les choses. Il est OK, on repart en salle d'accouchement pour tenter la chose. A minuit c'est l'échec : le bébé est toujours contre le col et ne descend pas.
On comprend maintenant qu'il faut accélérer les choses, sinon on va partir au bloc. On lui demande si on peut durant un moment pratiquer l'haptonomie pour aider le bébé à bien se présenter.
C'est notre dernier espoir. Ca fait 70h que le travail a commencé, on n'a plus d'autre solution. Julien, et Denise son assistante, se retirent et nous laisse tous les deux dans la salle. On trouve assez d'énergie pour se retourner, s'accroupir, respirer longuement, et entrer en communication avec Romane. Ca fonctionne ! On est très excité, moi je la sens bouger, mon petit coeur la sens pousser. On vis un moment d'une rare intensité malgré tant de fatigue.
Mais toujours pas de tête…
Mercredi 22 juin
Il est une heure. Julien et Denise reviennent. Malgré une heure d'hapto intensive Romane ne passe pas le col. Julien a attendu tout ce qu'il pouvait mais il n'y a pas d'autre choix maintenant que de partir au bloc pour une césarienne.
Mon amour remplie de désespoir fond en larme. .. 70h de travail pour finir en césarienne, c'est trop injuste. Denise la rassure avec les mots qu'elle trouve. Julien évoque sa grande frustration de laisser la place au chirurgien. Le calme et à la sérénité de l'hapto laisse la place à une agitation impressionnante : L'anesthésiste de garde qui vient faire l'injection, l'habillage stérile pour le bloc, les préparatifs autour du ventre.
